You can fill out and print these forms and bring them with you on the day of your appointment. Should you need to fill out the forms upon arrival, you will need to arrive 30 minutes prior to your appointment. Please bring an updated medication and allergy list, to include the strength and dose you take. Bring your insurance card(s) to include presciption card so that we can scan a copy to your chart. If you have AETNA and a referral is required, and do not have your referral at the time of your visit, your visit will need to be rescheduled because of the insurance requirements. If you cannot keep your appointment for any reason, please contact us at least 24 hours in advance to cancel. Please have your primary care doctor fax pertinent records to the office where you are to be seen.
